A Minnesota Health Care Directive or Wisconsin Power of Attorney for Health Care and Declaration to Physicians (Living Will), allow you to name someone to make decisions on your behalf in case you are incapacitated and specify your intentions for care under certain medical situations.

Emergency situations often present complex choices that can leave loved ones unable to act on your behalf.  By appointing a health care agent, you are able to communicate your health care preferences or end of life preferences in a straightforward, non-threatening manner.  A Health Care Directive is a crucial estate planning document to have in conjunction with your will or trust or when you are preparing for a trip, medical procedure, or dealing with a relative or loved one growing in age.
